To order Illinois Fraternal Order of Police license plates, please complete the following steps:

• Open and complete the Application and Affirmation.
• Print BOTH documents in their entirety and send, with full payment* to:

*Checks should be made payable to: Illinois Secretary of State

• Once received by the State Lodge, staff will verify that the applicant is a member in good-standing, sign the affirmation, and forward all necessary paperwork to the Secretary of State.
• Please note:
  • Incomplete applications and those sent without full payment will not be processed.
  • Associate members are not eligible for FOP license plates.
  • VANITY license plates are those that contain EITHER numbers OR letters; PERSONALIZED license plates are those that contain BOTH letters and numbers.
